Youth Case Manager Duties:

  • Work with school staff, families, and students to create a mutually responsive plan to address the needs the of the student that is preventing them from being successful in school.
  • Implement our services to address issues from lack of coping strategies, addressing negative behavior, not coming to school and/or being late, skipping classes, and low self-esteem – and many more.
  • Engage youth in their plan by creating goals and using their strengths and resiliency skills
  • Work with school districts, the county and other community-based agencies to provide services to students and/or their families based on their individual needs

Required Experience

  • A Bachelor’s degree preferred.
  • Experience in health, education, substance abuse, juvenile justice, or other related children and family services.
  • Experience working with youth and ability to engage youth in services preferred
  • Must be able to work independently and be flexible to high paced environment
  • Ability to track and input data through agency electronic systems
  • Ability and willingness to provide services in the home
